How to Soothe Sore Nipples from Breastfeeding and Pumping (What Actually Helps)

“A topic I would like to learn more about is how to help soothe sore and tender nipples from breastfeeding and pumping.” 

 

This question came straight from our Breezy Babies Lactation community, and honestly—it’s one of the most common (and important) ones I hear.

Sore, tender nipples can take something that’s supposed to be nurturing and turn it into something you dread. So let’s talk about what actually helps, because here’s the truth:

The fanciest nipple cream in the world cannot save a shallow latch.

 

Use Gravity to Your Advantage (Night and Day)

Spoiler alert: I love the side-lying position for a good deep latch the entire feed when you’re sleepy and dozing off in the middle of the night. Plus, you wake up so much more relaxed by laying down all night. It often makes it easier to stay comfortable, keep baby well-positioned, and maintain a deeper latch without constantly re-latching.

During the day, think about doing the same thing in a different way by leaning back and positioning your baby more on top of you. When baby is more belly-down on your body and you’re slightly reclined, gravity starts working for you instead of against you. This can help baby take in more breast tissue and stay in a deeper latch, rather than slipping onto just the nipple.

One more important detail that makes a big difference:
When you’re latching, move your baby toward their toes so they are looking up at your breast when they latch—not coming from the top or straight on. That upward angle helps baby open wider and take in more breast tissue, which is exactly what protects your nipples and makes feeding more comfortable.

Deep latch, good angles, and using gravity on your side can make a noticeable difference in both comfort and healing.

 

Where Nipple Cream Fits In

Nipple creams and balms absolutely have a place. They can soothe irritated skin, protect healing tissue, and reduce friction while things recover. But they are support—not the solution.

If the latch is shallow, no cream can outwork bad mechanics.

So yes, use the cream. But fix the latch first.

Pumping Pain? Let’s Talk Flange Size

When it comes to pumping, you’ve got to have the right flange size.

Most clients that I size for are not the standard 24 or 25 mm that comes with the breast pump. Most are between 11 and 17 mm. That surprises a lot of people.

Using the wrong flange size can cause pain, swelling, rubbing, blisters, and poor milk removal—and it can make pumping feel way harder than it needs to be.

WE RECOMMEND ORDERING THE SIZE YOU MEASURE (do not add 2–3 mm).

The right size should pull in just the nipple, maximize milk output, move comfortably in the tunnel, and not leave your nipple looking swollen or angry afterward.

  • A deep, comfortable latch, every time

  • Positions that help you stay relaxed, especially at night (think leaning back, or laying down)

  • Nipple cream to support healing, not to fix the root problem

  • A correctly fitted pump flange if you’re pumping

Breastfeeding and pumping should not feel like something you just have to power through. Pain is a sign that something needs adjusting—and that’s exactly what lactation support is for.
